Your “Fitness Grade” can be used to tell a great deal about your current physical state. It can and will also provide you a valuable set of benchmarks which you can use to measure your progress over time. Certain fitness grades are dedicated to your physical readiness while others can tell a great deal about your propensity for future diseases.
Complete the following PAR-Q to ensure you are ready to take on any fitness assessments as well as potentially start a new fitness program. When you appropriately answer each of the items in the PAR-Q move on and follow all directions in your self assessment.
You’ll then complete your self assessment and submit. Your responses will be evaluated by a fitness professional and your “Report Card” will be returned straight to your email inbox.
Complete that form and fax back to us at 425.460.8513 Or send it back through the drop box upload below. Your Fitness Grade and Report will be returned to you as an email attachment when complete.

Perform all assessments as directed and record.
Send assessment results to us via fax or the auto upload below.
